Hospital to home: the first 72 hours

Most readmissions trace back to three days. A short checklist for getting them right.

Reconcile medications the same day of discharge. Bring every bottle in the house to one table, including supplements, and match them against the discharge sheet.

Confirm follow-up appointments and transport before day three. A missed follow-up is one of the strongest predictors of readmission we see.

Set up the room for the body it has today: bed height, a clear path to the bathroom, a phone within reach of the chair.

Finally, name one person as the point of contact. Confusion between well-meaning relatives causes more missed doses than forgetfulness does.
